The 77003 Trap: When Fluoroscopic Guidance Is Billable

Why CPT 77003 is usually not reported with epidural and facet injections, where separate imaging can still apply, and how pain practices avoid denials and audit risk.

Short answerUsually not. Interlaminar epidural codes 62321 and 62323, transforaminal codes 64479–64484 and facet joint codes 64490–64495 include imaging guidance in their descriptors, so fluoroscopic guidance (77003) is not reported separately with them. Billing it anyway leads to denials and audit exposure.

CPT 77003 describes fluoroscopic guidance and localization of a needle or catheter tip for spine or paraspinous diagnostic or therapeutic injections. For years it was billed alongside spinal injections. That has changed.

Which injection codes include imaging

Code familyProcedureImaging included?
62321, 62323Interlaminar epidural, cervical/thoracic and lumbar/sacral, with imagingYes
62320, 62322Same procedures, without imaging guidanceNo imaging used
64479–64484Transforaminal epidural, by levelYes (fluoroscopy or CT)
64490–64495Paravertebral facet joint injectionsYes
64633–64636Facet joint nerve radiofrequency ablationYes

If imaging is used for an interlaminar epidural, the correct code is the "with imaging" version, not the "without imaging" code plus 77003.

Why this matters

  • Denials: payers and NCCI edits bundle 77003 into these codes.
  • Audit risk: routinely billing a separate imaging code on procedures that include it can look like unbundling.
  • Lost revenue the other way: using a "without imaging" code when imaging was documented underbills the service.

Where separate guidance codes can still apply

Some spine and paraspinous procedures don't include imaging in their descriptors. In those cases 77003 may still be reportable when imaging is documented. Always check the current CPT descriptor and NCCI edits for the specific code pair before billing.

A quick audit

  1. Search the last six months of claims for 77003.
  2. List the primary procedure codes billed with it.
  3. Check each against the current descriptor. Any pair where imaging is included is a fix.
